Kent Transition Charter

The Kent Transition Charter is an agreement between Kent County Council and all primary and secondary schools and colleges which sets out how schools, parents, local authority and other agencies will work together, with mutual respect, to support children and young people and their families as a child moves from one setting to another; Early Years to primary, primary to secondary, secondary to post 16 settings.

Kent County Council is working to ensure that the predicable needs of children and young people can be met at their local school. Therefore, this charter seeks to ensure that the best possible support is available for all children, including children with SEND, so the move to their new school is easy, children and their families feel confident that their needs are met and children will be successful in their new school.

This was shared at all of the Kent Countywide SENCO forums for 2021/2022.
