New Community Paediatric Referral Form

There is a new Community Paediatric Referral Form available.

There is a new Community Paediatric Referral Form available at the Kent Community Health website for schools in Maidstone, Tunbridge Wells, Dartford, Gravesham and Sevenoaks.

It is important that the form is fully completed by a GP, healthcare or educational professional and then sent electronically to kcht.CommunityPaediatrics@nhs.net.

Please be aware that incomplete forms will be rejected. GP’s will liaise with schools to ensure full completion as without sufficient information there is a risk the referral will be declined.

For reference, we accept referrals with the following criteria:

  • Developmental Delay or Impairment
  • Physical Disability, e.g. Cerebral Palsy, Duchenne muscular dystrophy
  • Assessment of children with suspected Autism or Asperger syndrome (a neurodevelopmental disorder) who are under 11 years of age*
  • Assessment of children with suspected ADHD between 6 and 11 years of age*. In case of significant concerns raised by school and parents, we may be able to start the assessment process between 5 and 6 years of age. However, we do not usually commence treatment until the child is 6 years old
  • Dyspraxia or co-ordination problems in Primary school children - 50 sessions of BEAM and 6 weeks of Clever Hands and Fingers to be initially undertaken at school. If concerns persist then refer to the Integrated Therapy and Care Coordination Service (ITACC).

*Children over 11+ years with suspected ASD or ADHD to be referred to CYPMHS (formerly CAMHS).
