Coming this August: new online EHCNA request form

Starting August 2025, all education settings will use our new online form to submit requests for an Education, Health and Care Needs Assessment (EHCNA).

What’s new?

  • You’ll now submit new EHCNA requests using a simple online form
  • Appendix 2 should still be completed as usual and uploaded through the online form.
  • This replaces the previous process of emailing Appendix 2 to KCC.

Key things to know:

  • This update applies to all education settings making new EHCNA requests.
  • Appendix 2 hasn’t changed - just upload it through the online form instead of emailing.
  • You don’t need SharePoint access to use the new form.
